What is Co-enzyme Q10?

COENZYME Q10- also known as ubiquinone, is a naturally occurring substance and an essential component of the mitochondria – the energy producing unit within our cells. CoQ10 is involved in the production of ATP, the energy molecule. As well as having a role in energy production and muscular performance, CoQ10 is also an antioxidant.

In sperm cells, coenzyme Q10 (CoQ10) is concentrated in the mitochondrial, where it is involved in energy production. It also functions as an antioxidant, preventing lipid peroxidation of sperm membranes. Supplementation with Co-enzyme Q10 in men experiencing infertility demonstrated a significant improvement in semen parameters; in particular, sperm count, motility and morphology. Additionally, a meta-analysis showed that Co-enzyme Q10 supplementation led to significant improvements in sperm concentration and motility.

Coenzyme Q10 is involved in the production of ATP, the energy molecule.Coenzyme Q10s antioxidant effects reduce lipid peroxidation and assist vitamin E in protecting cell membranes and lipids such as cholesterol from oxidation.

Most Coenzyme Q10 is synthesised in the body, however CoQ10 is also found in some foods. Meat, fish, nuts, and certain olive oils are some of the richest nutritional sources but the average dietary intake of CoQ10 is only 3– 6 mg per day.

Although the body makes Coenzyme Q10, deficiency can still occur and has been associated with several diseases. One of the major causes of CoQ10 deficiency is statin drug use which inhibits CoQ10 synthesis. As dietary sources are limited, supplementation is the most important means of improving CoQ10 deficiency regardless of the cause.

The absorption of CoQ10 is tripled by intake with meals and greater blood levels are maintained over a longer period of time. During absorption 95% of CoQ10 is converted into ubiquinol the reduced form of CoQ10, therefore the function of CoQ10 is not dependant on whether it is consumed as CoQ10 or ubiquinol.

Some dietary supplement companies claim that ubiquinol is superior to CoQ10, however this is unfounded. Currently there are about 11 human clinical studies of ubiquinol while there are over 300 clinical studies of CoQ10. The only clinical study directly comparing CoQ10 to ubiquinol for a health issue (dry mouth) found no difference in clinical effectiveness at an equivalent 100 mg dose of each. Other marketing claims have also been criticised such as the apparent lack of superior absorption along with the high cost of ubiquinol.

Is Co-Q10 good for Cardiovascular Diseases?

A number of intervention trials administering supplemental coenzyme Q10 at 100- 300 mg/day for up to three months to congestive heart failure patients’ demonstrated improvements in some cardiac function measures and other health benefits.10 Other human clinical trials have found benefits with CoQ10 supplementation in regards to cardiomyopathy, congestive heart failure, angina, and endothelial dysfunction. Another study investigating combined supplementation with 200 mg CoQ10 and 200 mcg of selenium daily over a 5-year period in older age individuals (aged 70 to 88) resulted in significant reductions in heart disease related deaths.15

Is CoQ10 good for high Blood Pressure?

An analysis of 12 clinical trials around cardiovascular diseases  investigating the blood pressure lowering effect of CoQ10 concluded that CoQ10 has the potential to lower systolic blood pressure by up to 17 mm Hg and diastolic blood pressure by up to 10 mm Hg without significant side effects. These reductions are comparable to conventional medications. Most clinical studies have used a dose of approximately 100- 150 mg per day of CoQ10, and no interactions between CoQ10 and blood pressure lowering medications have been observed suggesting it can be used safely alongside blood pressure medications.

Does CoQ10 help with energy?

Improvements in fatigue have been seen with a daily 150 mg dose.19 A recent study set out to see if a higher dose of CoQ10 would produce a better result. A group of healthy volunteers took either a placebo, 100 mg or 300 mg of CoQ10 for eight days and underwent a physical fatigue task (riding hard on a stationary bicycle) to see if the CoQ10 made any difference to their fatigue. The group taking 300 mg of CoQ10 reported greater physical performance, reduced subjective fatigue and greater recovery compared to placebo while the 100 mg dose had no real effect.

Can CoQ10 help with Fibromyalgia?

Therapeutic beneficial Effects of Coenzyme Q10 supplementation have also been noted in those with fibromyalgia. A case report review found a reduction in symptoms associated with fibromyalgia including other health benefits such as chronic pain, fatigue, sleep disturbances and depression when supplementing with 300mg CoQ10 daily for 9 months. Another case series found that supplementing with 300 mg of CoQ10 daily given in three divided doses resulted in marked clinical improvement in major symptoms including fatigue, pain and migraines. Additionally CoQ10 treatment raised blood CoQ10 levels and decreased oxidative stress.

Does CoQ10 improve sports performance?

Studies looking into athletic performance found a significant improvement in aerobic and anaerobic exercise performance when supplementing with 120mg of CoQ10 daily. A single dose of CoQ10 was found to increase blood levels of coenzyme Q10which significantly correlated with muscle CoQ10 levels, maximal oxygen consumption, and better performance (treadmill time to exhaustion), which further improved over 2-weeks. Other beneficial effects of Coenzyme Q10 was also been shown to reduce subjective fatigue sensation and improve physical performance in another study. Also, in a double-blind study CoQ10 was shown to reduce muscle injury in a group of kendo athletes and increased exercise capacity..

Can CoQ10 help with Periodontal disease

The authors also surmised that CoQ10 supplementation decreased pro-inflammatory cytokines and inflammatory markers in the elderly with low CoQ10 levels (Fan et al., 2017).Metabolic diseases, characterized by chronic, low grade inflammation, respond well to CoQ10 supplementation with significant decrease in TNF-α plasma levels without having an effect on C-reactive protein and IL-6 production (Zhai et al., 2017).

Periodontal disease is an inflammatory disease process resulting from bacterial attack. Periodontal pathogens can induce ROS overproduction and cause collagen and periodontal cell breakdown. Therefore Coenzyme Q10’s antioxidant action can protect against the damaging action caused by ROS and has shown to effectively suppress gum inflammation. Moreover, periodontal tissue biopsies have discovered low levels of Coenzyme Q10. Topical effects of Coenzyme Q10 has been shown to be beneficial and a small study has shown that CoQ10 supplementation was effective within 7 days.

Does CoQ10 improve male fertility?

In sperm cells, the majority of Coenzyme Q10 is concentrated in the mitochondria of the midpiece, so that the energy for movement and all other energy-dependent processes in the sperm cell also depend on the availability of Coenzyme Q10. In one study, infertile men were supplemented with 300mg of Coenzyme Q10 daily for 26 weeks, results showed significant improvements in sperm count, density, motility and morphology.

You should use CoQ10 if your on Statins?

Statins are frequently prescribed to lower cholesterol and may cause side effects by interfering with natural production of CoQ10. There is some evidence to suggest that starting on 100 mg per day supplemental effects of Coenzyme Q10 can reduce side effects in people sensitive to statins. Statins, a class of drugs used to lower cholesterol, can sometimes cause muscle pain.

Can CoQ10 help with headaches and migraines?

Brain energy metabolism has been found to be abnormal in people with migraine headaches, and a number of studies have suggested that by improving energy metabolism CoQ10 can reduce migraine frequency and severity. For example, Effects of Coenzyme Q10 supplementation for 3-months showed significant reduction in migraine frequency compared to placebo. And another study found that after 3-months CoQ10 reduced migraine frequency by 55.3%.

What are Medium Chain Triglycerides (MCT)?

MCTs are derived from coconut oil and consist of a short 6 to 12 carbon chain rather than the long 18-24 found in polyunsaturated fats (PUFAs). This difference in chain length means that they are metabolised differently. Unlike other fats, MCTS do not require pancreatic enzymes and bile acids to be absorbed. As CoQ10 is a lipid soluble nutrient MCTs act as a perfect carrier to enhance absorption and for energy production and metabolic efficiency.

 

Coenzyme Q10 and antioxidant protection

Present in nearly all cellular membranes, CoQ10 offers antioxidant protection, in part, by maintaining the reduced state of α-tocopherol (α-TOC) and ascorbic acid (ASC).

CoQ provides antioxidant protection to cell membranes and plasma lipoproteins (López-Lluch et al., 2010).

The antioxidant protection conferred by CoQ10 is associated with skeletal muscle performance during aging as evidenced by the fact that a high CoQ10H2/CoQ10 ratio is accompanied by an increase in muscle strength (Fischer et al., 2016).

It seems clear that supplementation with CoQ10 improves mitochondrial function and confers antioxidant protection for organs and tissues affected by various pathophysiological conditions.

While most people tolerate coenzyme Q10 well, it can cause some mild side effects including stomach upset, loss of appetite, nausea, vomiting, and diarrhea.

However, a 2-year treatment with CoQ10 (300 mg/day) as adjunctive therapy in a randomized, controlled multicenter trial affecting 420 patients suffering from chronic heart failure (Q-SYMBIO trial) demonstrated an improvement in symptoms and reduction in major cardiovascular events (Mortensen et al., 2014).
